I, the undersigned also state that I am in good physical condition and know of no reason why I cannot train in this form of
training. I have been advised that I should consult with a physician before participating any physical conditioning or martial
arts program. I certify that I have not been advised otherwise by a qualified medical person that I cannot participate in Jiu
jitsu. I certify that I alone am responsible for knowing my physical limitations and health conditions. I understand that Jiu
jitsu is a close-contact sport and that it is possible to contract illnesses (including but not limited to COVID-19) or
infections.
I understand that in case of emergency, I hereby authorize any licensed medical personnel to perform any excepted
medical assistance deemed necessary and I agree to bear the expense of any such treatment. As additional
consideration for training at the ACADEMY, I agree that my attendance and or performance at the Academy and abroad
at events maybe photograph, filmed and/or taped and used by the ACADEMY for marketing purposes and out the rise the
use of my image and I waive any compensation thereof-even if I discontinue my training at the ACADEMY. I acknowledge
that I would like to receive correspondence from the Academy via telephone, mail, and email. I, the undersigned, being
duly aware of the risks and hazards inherent upon participating in this form of training and martial arts agree to all its
rules, terms, and conditions. I knowledge that I have received a copy of the ACADEMY rules, have read them, understand
them, and agreed to honor and obey them. I understand that disobeying the rules may, at the instructor or owner’s
discretion, result in suspension or expulsion from the ACADEMY and that I will not be entitled to a refund, and if expelled
must pay the remainder of my contract.
In consideration of my application and permitting me to train at the ACADEMY, I hereby take action for myself, my
executors, administrators, heirs, next of kin, successors, and assigns as follows: A) Waive, release and discharge from
any and all liability for my death, disability, personal injury, property damage, property theft or actions of any kind which
may hereafter accrue to me or my traveling to and from events or tournaments in which I compete on behalf of the
ACADEMY, the ACADEMY and any of its directors, officers, and employees. B) Indemnify and hold harmless the entities
or persons mentioned in this paragraph from any and all liabilities or claims made by other individuals or entities as a
result of my or any actions during training or competition.
The Accident Waiver and Release of Liability shall be construed broadly to provide a release and waiver to the maximum
extent permissible under applicable law. I hereby certify that I have read this document and I understand its contents.
PARENT OR GUARDIANS FOR MINORS (UNDER 18 YEARS OF AGE)
The undersigned parent and/or natural guardian or legal guardian does hereby represent that he/she is, in fact, acting in
such capacity and agrees to save and hold harmless and indemnify each and all of the parties referred to above from all
liability, loss, cost, claim or damage whatsoever which may be imposed upon said parties because of any defect in or lack
of such capacity to so act and release said parties on behalf of the minor and the parents or legal guardian.
